New Student Registration
In order to be ready for ARENA SCHEDULING, all NEW students must submit the following REQUIRED
documentation between January 22 and April 30, 2025. After April 30th, 2025 all new students will
enroll in August by appointment only. Call 801-567-8530 to schedule that appointment.
 
Fill out this form and bring all the required information below:

ALL NEW students must submit the following REQUIRED documentation.
**Online Registration is required for all new students to Herriman High School.**
If ANY items are missing, your enrollment will not be completed
 
● Original Birth Certificate (we will make you a copy)
● Picture Identification from legal guardian
● Unofficial Transcript/Report Card – obtain from previous school
● Proof of Residency – 2 forms which must include 1) a lease agreement or mortgage statement AND
2) a current utility bill, bank statement, pay stub, etc) – showing CURRENT address
● A Permit/Open Enrollment Application is required IF address isn’t in HHS boundaries and must be
approved BEFORE we can proceed with registration. Permit applications can be accessed by going to
herrimanhigh.org or jordandistrict.org
● Immunization Record – MUST have:
○ 5 DTap, DTP, DT
○ 1 Tdap (TD Booster)
○ 4 Polio
○ 2 MMR
○ 3 Hepatitis B
○ 2 Hepatitis A (if born after 7/1/1996)
○ 2 Varicella (Chickenpox)
○ 1 Meningococcal
● Letter of Intent – if coming from a K-12 Charter school (Paradigm, Providence Hall, Utah Virtual
Academy, Summit Academy, etc.)
 
**Online Registration is required for all students new to Herriman High School**
-Please go to http://planning.jordandistrict.org/enrollment/
-If student is new to Jordan School District, select Enroll: New Account
-If student is currently or has ever been in Jordan School District, select Enroll: Existing Account
-Complete all of the steps on this application and upload all required documents listed above when prompted in the
registration process.

Si falta CUALQUIER artículo, su inscripción no se completará

 - Partida de nacimiento original (haremos una copia)

- Identificación con foto del tutor legal

- Transcripción no oficial / Boleta de calificaciones: obtener de la escuela anterior

- Prueba de residencia: 2 formularios que deben incluir 1) un contrato de arrendamiento o declaración de hipoteca Y 2) una factura actual de servicios públicos, estado de cuenta bancario, talón de pago, etc.) que muestre la dirección ACTUAL

- Se requiere un Permiso/Solicitud de Inscripción Abierta SI la dirección no está dentro de los límites del HHS y debe ser aprobada ANTES de que podamos continuar con el registro. Se puede acceder a las solicitudes de permisos visitando herrimanhigh.org o jordandistrict.org

- Registro de vacunas - DEBE tener:

  • 5 DTap, DTP, DT 3 Hepatitis B
  • 1 Tdap (refuerzo de TD) 2 Hepatitis A
  • 4 Poliomielitis 2 Varicela(chickenpox)
  • 2MMR 1 meningococia

- Carta de intención: si proviene de una escuela chárter K-12 (Paradigm, Providence Hall, Utah Virtual Academy, Summit Academy, etc.)

***También se requiere preinscripción en línea SI ES NUEVO EN EL DISTRITO ESCOLAR DE JORDAN. Si su estudiante ALGUNA VEZ ha sido estudiante en el Distrito Escolar de Jordan (K-12), NO es necesario registrarse en línea.
